In the HIM/Medical Records Department, we strive to address your information needs including chart prints, radiology images, electronic record online assistance, or other specific requests.

To obtain a copy of the medical record, the patient’s authorization must be signed, dated, and must include the name and address of the individual who is to receive the copies of the record, the information requested and the treatment dates. For MYRECORD users, log into your account from your Whitfield Regional account. From there you have access to view, download, print and request your medical record.
